Conclusions: Abnormal renal profiles (i.e., low levels of eGFR and high levels of MAU) were associated with the progression of DR, but not with the development of DME.

    • In an earlier cross-sectional study, this group of Chinese investigators confirmed the correlation between renal impairment and the presence of DR and DME in T2DM. In the current study, the researchers assessed three-year follow-up data to analyze the interplay between renal function (eGFR and albuminuria) and the development of DR and DME in 1409 T2DM patients (1409 eyes). Of this total study sample, 143 patients had DR progression and 54 patients developed DME.
    • Mechanisms tying microvascular dysfunction in the eye and the kidney include the presence of hyperglycemia leading to oxidative stress and the formation of advanced glycation end products and other pathological damage. These changes could result in adverse vascular remodeling that causes microvascular lesions in the eye and the kidney. Another possible mechanism is that vitamin D deficiency, which is common in patients with CKD, is related to DR.
    • Limitations of the current study include that there was a small sample size of patients with DME, and the study population was Chinese, with results less generalizable to other populations.
